You add questions to be asked upon registration by going to the Account tab of the admin navigation panel and clicking Edit Site User Profile Questions. Type the question in the text box and then select the kind of answer you'd like from the drop down menu, short answer, longer answer, multiple choice, etc. You can choose whether members see a question upon registration or when they are updating their public profile.
